What is PLAN?

PLAN Coordinates Quality Healthcare for Low-Income, Uninsured Collier County Residents

MISSION: To make a significant difference in the health of our community by providing a safety net and increasing access to quality healthcare for all low-income, uninsured adult residents of Collier County.

Originally launched by the Collier County Medical Society in 2003, PLAN is a community-based referral network program of 200 volunteer physicians, community clinics, hospitals and other affiliated health care providers who help to provide a safety net of access to quality healthcare for that underserved population in our community.

MEDICAL SERVICES

PLAN coordinates delivery of all volunteer medical care for qualified residents including consultations, procedures, surgeries and laboratory services through an integrated system. By helping people navigate the local healthcare system, we help them receive the medical care they need.

DISCOUNT PRESCRIPTIONS, LAB & IMAGING SERVICES

Through a partnership with the Collier County Housing and Human Services department, everyone is invited to print a card for discounted prescriptions, laboratory and imaging services by visiting Coast to Coast Rx.

I am a physician or representative of a medical care facility, and I would like to learn about the benefits of joining PLAN’s volunteer network.
