What We Do

Physician Led Access Network (PLAN) of Collier County is a 501(c)(3) non-profit organization that:

Coordinates Quality Healthcare for Qualified Low-Income, Uninsured Collier County Florida Residents Ages 19 to 64.

MEDICAL SERVICES
Our network of physicians, hospitals, and other healthcare facilities and providers offer free medical care for qualified residents including consultations, procedures, surgeries and laboratory services through an integrated system.

PRESCRIPTIONS, PHARMACY, LABORATORY & IMAGING SERVICES
Collier County residents may also be qualified to receive these free services through PLAN’s program.

CASE MANAGEMENT
By helping our participants navigate the local healthcare system, we help them receive the medical care they need.
