PLAN is a network of volunteer physicians, health clinics, diagnostic centers, hospitals, and many others who volunteer their services to help you get well and stay well. They are not paid for their services to you. This is not a government program or an “entitlement” program. The assistance available and other conditions of the program may change at any time.
Please note that PLAN does not cover emergency room expenses, ambulance, or medical supply services.
Who is Eligible?
- You must be a Collier County adult resident between the ages of 19 to 64.
- You must be able to provide proof of Collier County residency for at least 90 days prior to making application.
- You must not have health insurance of any kind, are not eligible for Medicare, or Medicaid; and have an immediate medical need.
- Your individual and family income must be below the 200% of Federal Poverty Guidelines (see chart below).
- You do not have to have U.S. citizenship.
- Enrollment is for a 12-month period subject to meeting program guidelines. An extension may be granted pending continued eligibility and continued treatment. However, it is your responsibility to call the PLAN Patient Care Coordinator to request an extension.

What services does PLAN offer?
A qualified PLAN patient may receive services that include*:
- Specialty healthcare services by participating PLAN members
- Limited diagnostic tests, laboratory work and imaging (x-rays, MRIs)
- Hospitalization
- Surgery
- Pharmacy Discount Card

How do I become a PLAN patient?
- You must be referred to PLAN by a current volunteer member of PLAN. This may be your primary care physician, another specialty healthcare member of PLAN or one of the community clinics or hospitals.
- If you are referred to PLAN, you will be instructed to contact one of our Patient Care Coordinators to make an appointment to apply to PLAN.
- Prior to your appointment with the Patient Care Coordinator, she will explain the type of documentation you will need to bring with you to the appointment when you apply.
- We reserve the right to verify information that you have provided in your application. If you have provided inaccurate or false information to us you may be required to pay for any assistance you have received. If we find you have intentionally misrepresented information, you will be terminated from the program.
What do I have to do as a PLAN patient?
If you are accepted into the PLAN program, you:
- Understand that all appointments are to be scheduled through a PLAN Patient Care Coordinator at (239) 776-3016.
- Keep each doctor’s appointment. If you miss appointments without letting the your primary care provider AND the PLAN Patient Care Coordinator know at least 48 hours before your appointment, we must terminate your relationship with the program.
- Will not schedule follow-up appointments with any doctor, clinic, or hospital. All appointments are to be scheduled through a PLAN Patient Care Coordinator at (239) 776-3016.
- Will follow treatment plans, for example: obtain prescribed medicines and take as directed.
- Will promptly supply any information which may be requested by the program.
- Will allow PLAN to share, as necessary, any information regarding your participation in this program with other PLAN of Collier County participating individuals, organizations and agencies.
- Will immediately contact the PLAN office if your income changes or if you become covered by Medicare, Medicaid, private insurance, other health insurance or medical benefits.
- Will apply for Medicaid or other assistance programs at our request.
- Will contact PLAN immediately with any changes in your address or phone number.
- Will, if you have an emergency illness or incident, go to an emergency room. Emergency room use is only for true medical emergencies and is not covered under this program. When you must visit an emergency room, immediately call the PLAN Patient Care Coordinator and your primary care clinic/enrollment site. Your follow-up is critical to getting well and staying well.
