Patient Eligibility

  1. Patient must be a Collier County resident for at least 30 days prior to enrolling
  2. Patient does not have health insurance of any kind, is not eligible for Medicare, or Medicaid; and is medically needy
  3. Income - under 200% of Federal Poverty Guidelines (see chart)
  4. Enrollment is for a 12 month period. An extension may be granted pending continued eligibility and continued treatment. However, it is the patient's responsibility to call their enrollment site/primary care provider or the PLAN Patient Specialist for an extension to be re-enrolled.
  5. U.S. citizenship is not required

Patient Responsibility


As a PLAN patient: Your Role in Getting Well & Staying Well

PLAN volunteer physicians, health clinics, diagnostic centers, hospitals, and many others are volunteering their services to help you get well and stay well. They are not being paid for their services to you. Please remember: This is not a government program or an "entitlement" program. PLAN does not cover emergency room expenses, ambulance, or medical supply services. Your responsibilities under this program, the assistance available and other conditions of the program may change at any time. We reserve the right to verify what you have told us. You may be required to pay for any assistance you have received based on inaccurate information you provided to us. If we find you have intentionally misrepresented information, you will be terminated from the program.

General Responsibilities


As a PLAN patient, you agree that you:
  1. Understand that all apointments are to be scheduled through PLAN of Collier County.
  2. Will not schedule follow up appointments with any doctor, clinic, or hospital. All appointments are to be scheduled through PLAN of Collier County.
  3. Will follow your treatment plan, for example: get prescribed medicines and take as directed.
  4. Will promptly supply any information which may be requested by the program.
  5. Will allow PLAN of Collier County to share, as necessary, any information regarding your participation in this program with other PLAN of Collier County participating individuals, organizations and agencies.
  6. Will immediately contact your enrollment site or PLAN of Collier County if your income changes or if you become covered by Medicare, Medicaid, private insurance, other health insurance or medical benefits.
  7. Will apply for Medicaid or other assistance programs at our request.
  8. Will contact PLAN of Collier County immediately with any changes in your address or phone number.
  9. Will, if you have an emergency illness or incident, go to an emergency room. Please remember, emergency room use is only for true medical emergencies and is not covered under this program. When you have visited an emergency room, immediately call the PLAN Patient Care Specialist and your primary care clinic/enrollment site. Your follow up is critical to getting well and staying well.

Referrals


You agree to:
  1. Keep each doctor's appointment. (If you miss appointments without letting the your primary care provider AND the PLAN Patient Care Specialist know at least 48 hours before your appointment, you will be dropped from the program.)
  2. Show your PLAN of Collier County patient ID card each time you see a doctor or other health care provider (if you do not show your card at each visit you may be billed for that visit).
  3. Call your enrollment site (Primary Care Clinic) / PLAN Patient Care Specialist if you need to be seen anywhere else for care.

Medications Assistance


If applicable, you understand that:
  1. Will receive a pharmacy discount card that can be used at most pharmacies in Collier County.
  2. You will be asked to apply for pharmaceutical company medication assistance programs when available.
